2) Browse through the search results and find your record. Click on your name to open the details of your record.
3) When your detailed record opens, copy your record URL indicated in the red rectangle in the image below.
4) Remain on the page with your record and scroll down to the bottom. Find the Opt Out link and click it.
5) A new window with an Opt Out form will open. Paste your record URL from Step 3 above into the field provided in Opt Out Step 1. Answer the security question and click SUBMIT.
NOTE: A security verification question may appear before Step 1 of your Opt Out. Complete the security question to continue to the Opt Out form.
6) Step 2 of the Opt Out form will appear. Enter your full name, email address, reason for your opt out, complete the security question, click the check box and click SUBMIT.
